Investigation, diagnosis and treatment for musculoskeletal conditions.

Rheumatology covers a wide variety of the musculoskeletal conditions involving the locomotor system e.g. joints, bones and soft tissues.

In our clinic here at BMI Albyn Hospital, we see patients with:

  • Inflammatory joint diseases
  • Auto-immune inflammatory connective tissue diseases
  • Osteoporosis and metabolic bone diseases
  • Gout and pseudogout.

The above list covers the broad classification of the conditions, with each being an umbrella term which describes a number of conditions.

Joint disease, for example, covers rheumatoid arthritis, psoriatic arthritis, spondyloarthritis, enteropathic arthritis and infection/post infection reactive arthritis. Inflammatory connective tissue includes lupus (systemic lupus erythematosus), scleroderma (systemic sclerosis), Sjogren’s syndrome, inflammatory muscle diseases (polymyositis and dermatomyosists), anti-phospholipid syndrome and mixed connective tissue disease.

Metabolic bone diseases include osteoporosis, Osteomalcia and Paget’s disease of bone.

Rheumatology also receives referrals of patients with non-inflammatory and mechanical musculoskeletal symptoms like repetitive strain injuries, over-use syndromes, chronic widespread pain, chronic fatigue syndrome, generalised aches and pains, fibromyalgia syndrome and joint hypermobility. However, beyond diagnosis and exclusion of other serious rheumatic disease there is not much the rheumatologist can do for these conditions, aside from recommending pain medication, physiotherapy, hydrotherapy, occupational therapy and/or a referral to chronic pain service if the pain is severe despite the aforementioned interventions.

A referral to a clinical psychologist may also be available. It is the inflammatory conditions which rheumatology can make a real difference to, says one of our consultants here at BMI Albyn Hospital.

Our consultant will take a full medical history, carry out an examination and if he is still unable to reach a diagnosis he will arrange necessary investigations like blood tests or imaging with X-ray, ultrasound or MRI scans. DEXA or bone density scans in Aberdeen can be arranged by one of our consultants too.

There is no long wait for investigations at Albyn Hospital and some ultrasound scans can take place in-clinic by one of our consultants. Ultrasound can sometimes lead to a quick diagnosis or exclusion of a diagnosis at the same visit, with no need to wait for other scans.

A bespoke treatment plan may then be offered, which can include medication and or a minor procedure performed on the same day, such as a joint injection or joint aspiration.

If we diagnose inflammatory joint disease, we prescribe medication, and/or joint injections (which can take place in the clinic and in the same sitting). If it’s a tricky joint to inject, or a deep joint, we sometimes refer our patients for injection under imaging.

We offer specific treatments for patients with gout and can treat a flare as and when it happens.

Advice and physiotherapy are also offered for patients who require it.

The waiting time is far shorter to be seen privately and we have access to a wealth of ultrasounds, which can be done in clinic.
