The questionnaire may seem long and some of the questions quite personal but please complete this as best you can as it will help us to tailor your treatment plan quickly. Your answers will only be viewed by clinicians involved in your care and your answers will be analysed within a few days.

You will then be contacted to arrange your initial appointment.
