There are many conditions that can affect the knee, ranging from sports injuries to arthritis which can affect people of all ages.

Knee arthritis - Knee arthritis is a common condition, and although it can affect people of all ages, it becomes more likely as we age. Patients who used to play a lot of sport or do a job that puts pressure on the knees may develop arthritis. There are many different forms of arthritis that patients may experience:

  • Osteoarthritis - the most common form of arthritis in the knee simply represents  wear and tear and ageing
  • Inflammatory arthritis, such as rheumatoid arthritis or gout
  • Post-traumatic arthritis develops after an injury

Common signs of arthritis include:

  • Knee pain and stiffness
  • Pain at night that can disturb sleep
  • A loss of function in the knee
  • An inability to bend down or walk easily

It is important to see a specialist if you are experiencing any symptoms related to arthritis, as early treatment can be most effective.

Knee meniscus (cartilage) tear - This is one of the most common knee injuries. It can happen when you are forced to twist or rotate your knee, for example, during sport. Common symptoms include pain, swelling and stiffness and an inability to extend fully.

Ligament injury - In the form of ligament injury, sports injuries are common causes of knee injuries in all age groups , often coming from field sports such as skiing, athletics, and netball.

Tendinopathy - Patients that develop pain in the knee from tendonitis (the tendons in the knee becoming inflamed).

When experiencing pain in and around the knee, seeing a specialist as soon as possible is a good idea so that you and your consultant can best assess and plan for any future treatment that you might need. Consultants will always opt for the most conservative therapy before resorting to procedures such as a knee replacement. So you can rest assured your consultant will work with you to treat your pain and discomfort in the most conservative way possible.

The goal of knee treatment is to use the most conservative and appropriate option for each patient. Patients may initially start with physiotherapy and anti-inflammatory medications to help treat the knee.

After these initial treatments take their course, injection therapy may be used to relieve inflammation and pain quickly and effectively. These injections include:

  • Steroid injections
  • Hyaluronic acid injections
  • Platelet-rich plasma (PRP) injections

Procedures include:

  • Knee resurfacing - this procedure addresses and targets the joint surface lining defects in the knee, improving pain and function.

  • Revision surgery - for patients that need to have an existing knee replacement revised because it has “worn out”.

  • Knee replacement surgery -  Patients can have either a partial knee replacement if the arthritis is not widespread, but the more common procedure is a total knee replacement to replace the entire joint. If you choose to have knee replacement surgery, the damaged or deteriorated part of your knee will be removed and replaced with an artificial joint. Not only can a new knee significantly reduce or remove your pain, but it can also help you to live life to the full again.

  • Knee arthroscopy - Also called keyhole surgery, consultants perform knee arthroscopy to diagnose and treat problems relating to the knee, most often, meniscal tears.. One benefit of this procedure is that your recovery time is typically short allowing earlier return to normal life.

  • Ligament reconstruction - Also usually performed arthroscopically to repair or reconstruct ruptured knee ligaments and restore stability to the knee.

Typically patients will spend 2-3 nights in hospital after a joint replacement and will be visited by a consultant and physiotherapist every day.

At your initial knee consultation, the consultant will first take account of your symptoms, how severe your pain is, and what activities it prevents you from doing.

Your individual lifestyle must be considered as part of your decision-making process. Your knee symptoms may prevent you from completing a round of golf, or it might be that you can't even walk upstairs without pain. When considering knee surgery, getting you back to the activity you want to do is an important consideration.

Most patients will usually require imaging tests following the initial consultation. These are some common scans and tests that your consultant may recommend depending on your specific symptoms and circumstance:

  • X-ray - Used to produce images of your knee to detect arthritis.

  • MRI scan - A magnetic field is used to produce detailed imaging of your knee. An MRI scan helps your consultant further investigate problems signified by your symptoms and examination findings.

  • Ultrasound - Sound waves are used to produce internal images of the affected area.

Once the test results are ready, you'll have a follow-up appointment with your consultant to discuss them. Whatever the next steps, we'll provide clarity and answer any questions you might have to put you at ease.
