Orthopaedics is a surgical specialty which deals with the accurate diagnosis, assessment and treatment of the joints, muscles and tendons, bones and other related structures. Hip surgery is a complex area and consultant hip surgeons will not only provide their expertise concerning surgery itself but will also provide their expertise in directing management to appropriate non-surgical treatments for their patients.

Due to the considerable demands the hip joint suffers over time and it’s role in transferring force from the legs to the body while needing to maintain a large range of motion, hip pathology is varied. Some categories include:

  • Developmental hip and pelvic disorders
  • Hip impingement syndromes
  • Tendon/ soft tissue injuries around the hip and pelvis
  • Degenerative hip disorders (osteoarthritis)

On many occasions, the most effective treatment for hip pain may be non-surgical. This may include watchful waiting or adapting activities for a period. Physiotherapy rehabilitation will often allow appropriate symptom management when adhered to for patients and is vital as part of any post-operative recovery plan. Physiotherapy and other non-surgical treatment for hip pain may include: 

  • Graded strengthening
  • Flexibility exercises targeting the hip joint or specific muscle groups
  • Manual techniques to mobilise the hip joint in physiotherapy sessions
  • Injections to the hip joint (guided with ultrasound by an experienced radiology consultant)
  • Shockwave therapy at the tendon insertions

For patients with a clear structural diagnosis that needs an intervention several forms of hip surgery exist:

  • Keyhole hip surgery (arthroscopic hip surgery including labral repairs and osteotomies to address hip impingement disorders)
  • Hip joint replacement surgery (including resurfacing surgery or full hip joint replacements as well as custom fit hip joint prostheses)
  • Revision hip joint replacement surgery

During a hip surgical consultation our patients will discuss the history of their complaint including:

  • Duration of symptoms
  • Onset including details of any trauma to the hip
  • Additional symptoms including clicking or clunking
  • Twenty four hour pattern to the pain
  • Specific limitations to activity due to the pain

An expert hip joint examination will commonly include:

  • Assessing with careful palpation any areas of tenderness around the hip
  • Observing any biomechanical faults in standing and lying
  • Assessing the hip range of motion
  • Screening the lumbar spine (lower back) for pathology
  • Special tests to stress tissues such as the tendons around the hip and the labrum

Following the initial evaluation of the hip problems, our hip orthopaedic surgeons will offer a full frank discussion regarding what the likely diagnoses are, what the likely treatments options may be, and the best way of managing the problem including the next steps. Next steps will usually involve investigating further with imaging which may include:

  • Specialist X-rays. 
  • CT scans – Excellent when a more detailed 3D representation of the pelvic anatomy may be required. 
  • MRI scans – Hip musculoskeletal/ orthopaedic problems will often be diagnosed very reliably by this form of imaging. 
  • Ultrasound scans – Excellent for assessing soft tissue structures around the hip and has the ability to assess these tissues under hip movement.

Usually a combination of those modalities will be used to guide the most accurate diagnosis. Post imaging the value of seeing a specialist is that they can then interpret the scans with their patient (by matching symptoms with the scan findings) to come up with a logical diagnosis.

Unique to our BMI The Cavell Hip surgery team is that our specialists can offer patient-specific hip joint implants for our total hip replacement operations. Traditional off the shelf joint implants follow a one size fits all philosophy and have been used for forty to fifty years. With these types of prostheses there are compromises to be made due to the generic shape these implants offer; Often, traditional implants behave less like the original joint did due to a mismatch between the patient’s own anatomy and biomechanics and that of the implant. However, custom fit hip joint replacements are created by taking precise measurements for each patient with unique 3D CT scanning technology ensuring a perfect fit. Due to this fit, made to measure hip replacements have a better functional outcome for patients and have a reduced post operative complication rate.
